I keep hitting veins with my Lantus and almost dying. Am I doing something wrong?

I've only been diabetic a short time. In that time, I've injected my lantus into my legs and hit a vein 3 times. First time, I bottomed out at 28 before I started going back up. Last time, I bottomed out at 42, and today, I caught it at 48. I rarely hear of others having trouble with this. Am I doing something wrong? I have started taking 2 doses of Lantus instead of 1 to minimize risk, and I've been injecting into my buttcheeks.

I'm planning on getting on a pump soon, does anyone have problems with this on a pump?
