A1C of 12 help

Hey Y'all, Just got back from my doctor and got my A1C results and it was a 12.1, I know it's high and I told my doctor I didn't really take my medication or exercise, that I work long hours and as such my diet is shit. She wants to send me to an endocrinologist to get put on medication, and I advised against that, as I am an active Law enforcement officer and also I am about to finish my flight lessons and obtain my private pilots license,and having an insulin prescription on my record would screw me in both careers. Anyone here have any experience bringing down A1C levels from 12 or higher to 7 or close? Obligatory throwaway because duh. Edit- I am gonna start working out an hour a day, eating healthy, stop drinking 3 McDonalds coffees a day and also sleeping 7 hours a day. Edit- I am 5' 8", 225 pounds. Edit- I am currently on 2000mg of Metformin and one januvia pill a day
