Hi T1D for 20 years largely moderately controlled, had a1c approaching 7 before they stared to creep back up, i am generally pretty good, i have various tricks and solutions developed to problems, as im sure most of you have. Im stuck on one thing thats the cause of stubborn a1c.
Nocturnal hyperglycemia - If i go to bed with any kind of food in my stomach , carby or protein, it doesnt seem to matter. my suagrs will take off as soon as im asleep. My Cgm basically goes skyward shortly after bedtime. ive even noticed sometimes after a long nap they creep up. The only consistent way I have acheived stable over night sugars is by fasting, either one meal a day fasting (breakfast to breakfast) or full on water fasts for a couple of days.
If im working nights its not a problem if i eat i get a reonable bolus response, i will infact have a hypo around 2 am on nights. And if im busy at work during the day or late sugars are flat if not drifting downward. Suggesting my basal is adequate if not a bit too much.
This is basically every night for the past 5-6 years, with occasional relief by fasting or excessive dosing waking in the middle of the night, or setting a square wave for sleeping hours.
Has anybody experienced this? My endo team cant give me straight answers.
Its peculiar - sleep with food in stomach, regardless of dosing or what was eaten, cgm shows sugars take off around the time i fall asleep.
